Copied!

Created: 8/25/2026

Append-only state machine for each hypothesis in the case file (or meta-case). State transitions gated by PCE-03 thresholds AND explicit steward approval on every transition (Q3 = require-approval-always; Ibrahim-confirmed 2026-08-25). Transition framing for medication/dosage/regimen follows the Dr House footer-only disclaimer rule (no prose-level weaving; cycle-level footer handles it). Multilingual: en/de/it/zh. Listed on Bazaar.
